Case management is a long term solution for elders who benefit from ongoing care planning, oversight, support, and management. Case management is often ideal for elders with busy or long distance caregivers, or elders with limited family support. Case management may include:

Consultation Services
Consultation services are typically a short term solution, ideal for adult children or caregivers who may feel overwhelmed or at an impasse with how to proceed with care. Clients may request consultation services for any reason, but requests are often related to the following:

Planning related to cognitive changes and/or dementia

Senior housing choices

Understanding healthcare reimbursement/Medicare and other benefit programs

Obtaining appropriate referrals to community resources

Support, education, and planning during a care transition, such as during a hospital stay

Care planning for both short and long term goals

Community referrals

Assistance with important health documents

Facilitating family meetings

Education related to safe transportation options

Assistance selecting an alternate housing option
