Care Coordination

Care coordination includes ongoing support, regular visits, advocacy, and communication with family. It's ideal when families live far away or for anyone who likes knowing that a licensed professional is monitoring their loved one. Care coordination includes:​

  • Monitoring through monthly, biweekly, or weekly home visits

  • Regular communication and updates with family

  • Accompaniment to medical visits when requested, coordination of medical care, advocacy

  • Collaboration with care providers and accountability for quality care

  • Access to professional care coordination software

  • Email response within 24 hours for immediate professional support


Project Packages

Project packages are typically a short term solution, ideal for people who may feel overwhelmed or at an impasse with how to proceed with care. Requests are often related to the following:​

  • Planning related to cognitive changes and/or dementia

  • Senior housing choices

  • Understanding healthcare reimbursement/Medicare and other benefit programs

  • Obtaining appropriate referrals to community resources

  • Support, education, and planning during a care transition, such as during a hospital stay
